If you don't have health insurance, or you're choosing not to use it for a specific service, you have the right to a written estimate of your expected costs from SEVA Healthcare before your appointment. It's called a Good Faith Estimate.

What it is

A Good Faith Estimate, in plain English

A Good Faith Estimate is a written document that shows what your medical care at SEVA Healthcare is expected to cost — before you receive it. It's part of a federal law called the No Surprises Act, designed to help patients plan for care and avoid unexpected bills.

Who is eligible

Any patient who does not have health insurance, or who has insurance but is choosing not to use it for a scheduled service, is eligible for a Good Faith Estimate.

Why patients receive it

So you understand your expected costs in advance and can plan for your care with confidence — no surprises when the bill arrives.

When to request one

Ask as soon as you're scheduling a non-emergency service, or even while you're deciding. The earlier you ask, the more time we have to prepare an accurate estimate.

Your rights

What patients are entitled to

Under federal law, if you're uninsured or paying out-of-pocket, you have the right to:

  • Receive a written estimate before scheduled care

    For non-emergency services, you can expect a written Good Faith Estimate before your appointment.

  • Request an estimate any time — even before scheduling

    You don't need to have an appointment booked. Ask us for an estimate while you're still deciding.

  • See your expected medical costs

    Your estimate will describe the services that are expected to be provided and the charges associated with them.

  • Keep a copy of your estimate

    We will provide your estimate in writing. Save a copy or take a photo — you may need it later if you have questions about your bill.

  • Federal protections if the bill is significantly higher

    If your final bill from the same provider or facility is meaningfully higher than the estimate, federal regulations may allow you to dispute the charges. Contact our billing department to discuss your options — see the Contact section below.

Your estimate covers

What's included in a Good Faith Estimate

Your written estimate will bring together the expected charges for the services scheduled for your care. Depending on the procedure, these may include:

  • Physician fees

    Charges for the doctor's professional services.

  • Facility fees

    Use of the surgery center, operating room, or clinic space.

  • Surgery / procedure costs

    Costs directly related to the scheduled procedure itself.

  • Imaging

    X-rays, MRI, CT scans, or ultrasounds related to your care.

  • Laboratory services

    Blood work, pathology, and other lab tests ordered for your visit.

  • Anesthesia

    Anesthesia services when clinically required for your procedure.

  • Other expected services

    Medications, supplies, and other related items reasonably expected for your care.

Actual services delivered may vary based on your clinical needs. If your care plan changes, we will provide an updated estimate whenever possible.

    Tell us the service you're considering — or that has been recommended — and any timing preferences.

  3. Our billing team prepares your estimate

    We assemble expected charges based on your planned services and send you a written estimate.

  4. Receive your estimate before your scheduled service

    You'll receive your Good Faith Estimate in writing before your visit whenever the timing allows.

Who qualifies for a Good Faith Estimate?

Any patient who does not have health insurance, or who has insurance but is choosing not to use it for a scheduled service, qualifies for a Good Faith Estimate under federal law.

Is a Good Faith Estimate the same as a final bill?

No. It is a written estimate of expected charges based on the services you are scheduled to receive. Your final bill may vary based on the actual services provided and any clinical changes during your care.

What happens if my treatment changes?

If your care plan changes and expected charges are different, we will issue a new Good Faith Estimate before the change takes effect, whenever possible.

Can insured patients also request an estimate?

Yes. While the federal Good Faith Estimate rules apply specifically to uninsured and self-pay patients, insured patients can still request an estimate of expected costs. Please contact our billing office — we're happy to help.

What if my final bill differs from my estimate?

If your final bill from the same provider or facility is significantly higher than the Good Faith Estimate you received, federal protections may apply — including a formal dispute process. Please contact our billing department so we can walk you through your options for your specific situation.
